Students registering for Athletics, Cheerleading, and Team Managers must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 10 school days prior to the first day of practice/tryouts.

Everyone is required to turn in a Health History Update Form and do the Health History COVID-19 Questionnaire. Once these are done, then the student can attend voluntary practices.

ALL students interested in participating in PGHS Athletics must have an up-to-date Pre-Participation Physical Examination Form. The pre-participation form must be completed utilizing the form provided on the PGHS Athletics website.
